New Patient Registration

The doctors welcome new patients.

As it often takes some time for records to be forwarded from your former practice all newly registered patients will be asked to complete a health questionnaire and are offered a consultation with the doctor. 

Medical treatment is available from the date of registration.  Please contact reception for further information.

Registering with the Practice

When you register you will also be asked to fill out a medical questionnaire (see below). This is because it can take a considerable time for us to receive your medical records. You will need to fill out the GMS1 form and the questionnaire and send completed forms to dansonfamilypractice@nhs.net

Named Accountable GP for all of our patients

GP Practices are required to inform patients that they have a named GP who is responsible for their overall care at the practice, please contact the practice if you wish to know who this is or if you have a preference and we will do our best to accommodate your request.
